The New Mexico-Mexico border region is one of the fastest growing and poorest areas in the United States. Access to health care is limited by the rural nature of the area, inadequate transportation, limited resources, low income, and low formal education levels of the border population. Given the lack of access to medical care, border residents infected with HIV are often diagnosed late. Their seropositive status is often detected during an inpatient hospital stay or at a sexually transmitted disease clinic. Most border clients are late to begin care and as a result their treatment regimens are not as effective. This continuation proposal is entitled the "New Mexico Border Health Initiative" and has four major goals: (1) Continue the Health Outreach Worker Program to increase the identification, testing, and enrollment of MSM, IDU, and women-at-risk for HIV infection into HIV services in southern New Mexico; (2) Increase access to HIV primary care and social services for clients living with HIV/AIDS in southern New Mexico; (3) Support HIV primary care for clients living with HIV/AIDS in southern New Mexico; and (4) Enhance the quality of HIV primary care for clients living with HIV/AIDS in southern New Mexico. To achieve these goals, the New Mexico Border Health Initiative proposes to engage in the following activities: (1) Utilize trained Health Outreach Workers to conduct outreach using culturally appropriate techniques; (2) Provide linkages to HIV primary care and social services for CdV clients; (3) Provide specialized disease management education to CdV clients; and (4) Provide training and education to the Community Health Center providers.

Camino de Vida Center for HIV Services is the lead agency. Michelle Valverde, Ph.D. will provide the external process and outcome evaluation services. The New Mexico Department of Health will provide counseling and testing training and early intervention nursing support. Medical care will be delivered by Ben Archer Health Center, La Clinica de Familia, and Memorial Medical Center's Family Practice Residency Program.

Five hundred prospective clients will be contacted and 250 will be tested annually. The project will continue to document all activities with the aim of providing HRSA with a replicable model that may be shared with present and future grantees. Close contact and cooperation with HRSA, the Centro de Evaluación, and the four other HRSA grantees will continue for the duration of the grant.
