Data Entry and Data Forms

To send data to the Evaluation Center please select one of the following:

1.  Print paper forms and either mail or fax to the Evaluation Center, or
2.  Use Adobe Acrobat to enter data and submit that data electronically to the Evaluation Center (this form looks like the paper form), or
3.  Use your web browser (Internet Explorer 5 or higher only) to enter data and submit that data electronically to the Evaluation Center.

  Process Submission method

Paper Forms

To enter data using paper forms, you either (1.) print the form from your computer if you have Adobe Acrobat loaded on your local computer, or (2.) contact the Evaluation Center to have paper forms mailed to you.

When completing paper forms, please use a medium point, black ink pen. Please write neatly within the boxes.

Forms can be faxed to the evaluation center on a 24 hour--7 days per week basis.

The disadvantage of paper forms is that there will be a higher data entry error rate.  (You can make a copy of the paper form before you mail it, but that is not necessarily the same data that gets entered into the multi-site database.)

Adobe Acrobat Forms

To enter data by using Adobe Acrobat PDF forms, you must have Acrobat Reader loaded on your computer. Click on the link to bring up the appropriate data form--then complete this form on the computer--and click on the "submit data" button on the bottom of the first page.

After clicking on the submit button, your data will be ready to be entered into the multi-site data set within approximately one minute. Staff at the Evaluation Center will review your data if TELEform informs us that there is a possible error.

Acrobat forms can used to send data to the evaluation center on a 24 hour--7 days per week basis.

The advantage of Acrobat forms is reduction of data entry errors, provides the sender with both printed and file copy of the actual data entered into the database, and provide the ability to analyze data within a few hours of submission. Additionally, data can be entered in the "field" on notebook computers and submitted to the Evaluation Center at a later time when connected to the internet.

Web Page Forms

To enter data by using a web browser, you will need to click on the form link. The only web browser that works is MS-Internet Explorer 5 (and higher). The HTML forms will NOT work with Netscape. If you get an error message, it is probably because you are using Netscape.

Using HTML forms are the same as Acrobat forms, except that the form does not look like the paper form.

HTML forms can used to send data to the evaluation center on a 24 hour--7 days per week basis.

The advantage of HTML forms is reduction of data entry errors and an ability to analyze data within a few hours of submission. The forms can also be printed prior to submission of the data, however, the printing is not a good as with Adobe Acrobat forms.
